CONSULTATIONS

WE OFFER THE FOLLOWING SERVICES:

• Personal health evaluation with individualized Wellness and Nutrition Plan for every client

• Telephone, email or local face-to-face consultations

• We also make custom-blended herbal teas, tinctures, salves, oils or lotions developed for your specific needs or we can recommend one of our in-stock name-brand products.

• Overall health, herbal & nutritional consultations available with Debbie Markel, CH, CNHP 

• Pet and people health consultations are available with Janice Goldstein, RN, MH.

CONSULTING FEES WITH DEBBIE MARKEL, CH, CNHP

     (Note:  Fee may vary with other consultants, so please call first.)

• $90.00 for the initial evaluation after receipt of Health History Form (see below).

• Additional charges for purchases or custom-blended teas, salves, oils, lotions or tinctures.

INSTRUCTIONS

A completed Health History form AND a 4 day detailed food diary (use whatever paper you want, but write down EVERYTHING you eat and drink) is required before a consultation with Debbie Markel,  

Print or download the health history form, then complete and mail or fax it to us with your food diary, your name, address and phone number.  She'll contact you to verify payment and ask further questions.  

Our fax number is the same as our phone number, so call first so we know a fax is coming through (804-598-5352).
